IMPORTANT HEALTH COVERAGE TAX DOCUMENTS NOTICE

Employco prepares Form 1095 for the reporting year for all Applicable Large Employer (ALE) Clients. Form 1095 provides information about the health coverage offered to you and, if applicable, your dependents.

Paper copies of Form 1095 will not be automatically mailed to employees unless required by state law. However, copies are available through your Employco Employee Portal.

To request a paper copy of your Form 1095, please use one of the following methods:

Requests must include your full name and your date of birth. Forms will be mailed to your home address within 30 days of your request.
